Living Well with Mobility Limitations
Mobility Limitations can be a risk factor for several conditions and lead to increased isolation and loneliness. A carer helps to make your home more accessible.
The Positive Aging Tool (PAT) is an online self-screening tool that offers a proactive approach to wellbeing, enabling early intervention and general health promotion to prevent often irreversible declines that can happen suddenly and seemingly without warning.
PAT was a 2020 winner of innovAGEING National Awards for realising wellness and reablement by providing “tailored resources to ensure older Australians are informed and in control of their ageing journey”.
Created by the Victorian-based not-for-profit Benetas in conjunction with Monash University, PAT allows users to take a six-monthly health check and then offers evidence-based recommendations to improve health and wellbeing.
The health check consists of five easy yes-no questions, which generate a tailored report with an option to download and work through the suggestions with the user’s GP.
Advice ranges from dietary and physical exercise recommendations to suggestions for home care services like those that InPlace Care provides.
You can take the PAT health check at the following link: https://www.benetas.com.au/pat
You can also get the Support you need at InPlace Care and live a healthy, happy life.
